Job TitlePopulation Health Fellowship
Job DescriptionJob Type: 
This is an opportunity for healthcare professionals interested in population health and passionate about health inequalities. The fellowship targets early to mid-career healthcare professionals providing NHS services (AfC band 6 and above, or equivalent; dentists-in-training; doctors-in-training post-FY2 and their SAS equivalent). The aim of the fellowship is to develop a network of professionals from a non-population health background with population health skills to benefit place-based healthcare systems across England.
It is a one-year part-time programme at two days a week alongside your substantive post. Population Health Fellows will work on a population health related project and will be supported by a blended virtual learning programme.  Supervision for the project will be provided. 
Applications are invited for the position of a Fellow in Population Health, funded by Health Education England, and based in Sutton Primary Care Networks Health & Wellbeing Hub, to commence the programme on 6 September 2022.  
Reimbursement of your salary element of the participation in the fellowship will be provided to your substantive employer and you must discuss this opportunity with your line manager and educational supervisor (if applicable) of your intentions to apply.

Working pattern:
There will be 2 fellowship days every week. One of the days will be a Tuesday, which is a compulsory fellowship day that cannot be changed. Learning events will coincide with this day (Tuesday) thus ensuring all fellows will be available to attend the learning together.

Job TitlePCN Pharmacist
Job DescriptionJob Type:  The post holder will be responsible for supporting the Sutton Primary Care Networks to provide expert medicine management and optimisation, vaccinating and drawing up vaccines, whilst working within their area of clinical competence and as part of a multi-disciplinary team. They will be supported by a Senior Clinical Pharmacist and others who will develop, manage and mentor them. The role will be patient-facing and pharmacists will be involved in structured medication reviews, including poly-pharmacy reviews, managing chronic long term conditions and managing medications after transfer of care. They will also have access to appropriate clinical supervision. The post holder will provide support and deliver patient services as determined by the Network Policy as well as local and national guidance. They will also provide leadership on quality improvement and clinical audit, manage some aspects of the Quality and Outcomes Framework and maximise cost-effective prescribing to improve the quality of patient care. The post holder will be enrolled on to the 18-month Primary care pharmacy education pathway with the Centre for Pharmacy Postgraduate Education (CPPE). They will also be supported to develop their role to become a non-medical prescriber, if that qualification is not already held. Salary: Competitive   Closing date: 15th September 2022
