Clinical Pharmacy Team

Our Pharmacy team consists of a range of pharmacists with combined background experience throughout the sector. Their aim is to utilise their skills in medication safety and medicines optimisation to support and promote the safe and appropriate use of medicines across our area. 

They work alongside primary care teams to review repeat prescriptions, answer patient queries, action clinical letters and carry out medication reconciliation following hospital discharges. They also liaise with specialist hospital teams when required and action any safety alerts that may appear. They also manage various long-term conditions ranging from hypertension, diabetes, asthma, complex polypharmacy and chronic pain reviews.

Some of the team members are Independent Prescribers and will utilise this skill in their specialist clinics.

First Contact Physiotherapy

We have a primary care based Physiotherapy service which allows patients, who are contacting their GP surgeries with musculoskeletal pain to be seen quickly by a specialist musculoskeletal physiotherapist. The physiotherapists’ will assess, diagnose and provide information on appropriate individualised rehabilitation and management plans. They will work closely with our clinicians to ensure patient’s get the best level of care and assist with any medication, investigations and referrals when deemed necessary.

This service is provided by Pure Physiotherapy and is supported by our local GP Federation One Health Lewisham.

Extended Hours

​GP practices are working together to offer patients better access to GP appointments and to provide additional appointments outside of their normal opening hours. These are known as Extended Hours appointments and are available during Monday to Friday; early morning or late evening. Please contact your local GP practice or visit your practice website for further information. 

There are now a range of ways you can get advice from a GP or practice nurse, including online consultations, telephone consultations and via the NHS App. To find out more or to book an appointment, speak your GP practice receptionist or a member of the practice team.
